This form serves the purpose of identifying students whose parents are interested in the PreK & Kindergarten Dual Language Program. In order to ensure that all applicants are considered, all information in this form must be filled out and parents/legal guardians must submit this form by the submission deadline. Please note that submission of this application does not guarantee placement in the program. The submission date will be a factor in the placement of students. *Priority will be given to students with siblings currently in the dual program.

Esta forma cumple con el propósito de identificar a los estudiantes cuyos padres desean que participen en el Programa de Lenguaje Dual de PreK & Kinder. Para asegurar que todas las solicitudes sean consideradas, los padres/tutores legales deben someter esta forma para la fecha límite de presentación. Es importante entender que el someter esta solicitud no garantiza un lugar en el programa. La fecha de sumisión será un factor para determinar la colocación de los estudiantes. *Se dará prioridad a los estudiantes con hermanos actualmente en el programa dual.
